Intake Process – Georgia Pediatric Program (GAPP)
At Pediatric Program, we understand that starting care for your child can feel overwhelming. That’s why we make the intake process simple, supportive, and family-friendly. Our goal is to guide you through every step so your child can receive the care they deserve at home.
Step 1: Initial Contact
Families reach out to us through our Contact Form, phone, or email. Our care coordinators listen to your needs, answer questions, and explain how GAPP works.
Step 2: Eligibility Screening
We perform an initial eligibility assessment based on your child’s age (under 21), medical condition, and care needs. If your child meets GAPP criteria, we move forward with the intake.
Step 3: Medical Documentation
You will provide your child’s medical history, treatment plans, and physician documentation. This ensures that we create a personalized plan of care that meets GAPP and Medicaid requirements.
Step 4: Care Plan Development
Our nursing team designs a customized care plan that includes skilled nursing or unskilled support services, depending on your child’s needs.
Step 5: Approval & Authorization
We assist in submitting all documents to Georgia Medicaid for review. Once approved, your child is officially enrolled in the GAPP program.
Step 6: Caregiver Assignment
We match your family with a qualified nurse or caregiver who has experience in pediatric home care.
Step 7: Care Begins at Home
Your child starts receiving safe, compassionate, in-home care. We monitor progress, adjust care plans when necessary, and remain available for ongoing family support.
